Concentrated H2SO4 has a specific gravity of 1.84, is violently exothermic on contact with water (dissolution releases 880 kJ/kg, sufficient to boil water on contact), and causes immediate coagulative necrosis with charring of skin, eyes, and mucosal tissue. The primary injury mechanism in drain cleaning is splash-back: when concentrated H2SO4 is poured into a partially water-filled drain, the exothermic reaction can cause explosive boiling and ejection of hot acid onto the user's face, arms, and torso. AAPCC reports over 3,500 drain cleaner exposures annually, with sulfuric acid products disproportionately represented in serious injury cases requiring hospitalization. Full-thickness chemical burns from H2SO4 produce deep eschar formation with ongoing subdermal penetration — surgical debridement and skin grafting are frequently required.
