Hydrogen peroxide systems (Clear Care, AO Sept) require complete neutralization through catalytic platinum disc decomposition over a minimum 6-hour period; premature lens insertion before neutralization causes severe chemical burns with immediate intense pain, tearing, and corneal epithelial sloughing. PHMB-based solutions have been epidemiologically linked to Acanthamoeba keratitis outbreaks — the 2007 Complete MoisturePlus recall by AMO (Abbott Medical Optics) followed a CDC investigation demonstrating a 7-fold increase in Acanthamoeba keratitis associated with the product, despite PHMB meeting ISO 14729 antimicrobial performance standards.
